Step 2: Submit Your Application Head over to Optimum's ACP page and provide your ZIP code and email address. Click on 'Apply Now,' and you'll be directed to the Optimum ACP application form. Fill in all the required information and attach the necessary documents. Finally, submit your application.
Step 3: Await Approval Once you've submitted your application, please allow a few days for Optimum to review it. You will be notified of your application status shortly. Once approved, you can start enjoying the benefits of Optimum ACP.
What is the Monthly Credit Amount?
Eligible households can receive a monthly credit of up to $30 (or up to $75 per month for residents on Tribal Lands) to be used towards their mobile or Internet service.
Who is Eligible for the Affordable Connectivity Program?
Both new and existing Optimum Mobile customers can take part in the program! You can qualify if you or someone in your household:

Receives SNAP/food stamps, Medicaid, or is eligible for another one of the FCC's Lifeline qualifying programs.

Receives benefits from the Special Supplemental Nutrition Program for Women, Infants, and Children (WIC).

Receives free or reduced-price school lunch or breakfast through the National School Lunch Program (NSLP).

Is enrolled in a school that offers free school breakfast and lunch to all students through its participation in the Community Eligibility Provision (CEP). Most New York City public schools qualify.

To receive your benefit through Optimum Mobile, you need to first confirm your eligibility via the National Verifier at GetInternet.gov. Considering the eligibility requirements, there is a possibility that my dependent is eligible for the Affordable Connectivity Program. Can I submit an application using my dependent's name?
The eligibility criteria set by the Federal Government can be applied to you or any member of your household, including dependents living with you. If your dependent meets one of the Federal Government's eligibility criteria, you can apply to the National Verifier as the primary applicant and designate your eligible dependent as the person qualifying for the benefits. However, please note that Federal Government rules prohibit an individual from being listed as the benefit-qualifying person on more than one Affordable Connectivity Program approval or enrollment.

During the application, you'll be required to provide your Optimum Mobile account number, Optimum Mobile phone number, and the National Verifier application ID, along with your name, address, and other necessary information exactly as they appear on the National Verifier approval.
